CHECK-IN INSTRUCTIONS:
- Check in after 4:00 PM
- The home is accessible via keyless entry. The code is 1973
- Please inform the Guest Contact of any problems or concerns upon arrival, so we can work to address these immediately

CHECK-OUT INSTRUCTIONS:
- Check out before 11:00 AM
- Prior to your departure, please tidy up the cabin by doing the following:
- Collect all trash and dispose of it in the trash can in the garage, then cover the can and lock the garage door (garage door key will be found on kitchen counter)
- Leave garage door key on kitchen countertop
- Strip linens from all used beds
- Adjust living room wireless thermostat to 55 degrees Fahrenheit (see instruction manual)
- Set living room small round gold thermostat to 50 degrees Fahrenheit
- Turn off all baseboards
- Unplug all small appliances
- Turn off all lights, electronics, and appliances
- Close and lock all windows and doors
- Additional fees will be imposed if the cabin is found in an unsatisfactory condition, so please return it to the condition in which you found it

HOUSE RULES:
- No smoking
- No pets allowed
- No events, parties, or large gatherings
- Must be at least 25 years old to book
- Please observe quiet hours after 10:00 PM. All exterior lights must be turned off at this time as well
- No open fires
- No trash can be left outside. All trash must be stored in trash bags and cans in the garage only
- Garage is not for guest use, and the garage door must be kept closed at all times
- Doors must be locked when not at the property
- Please report any damages that occur to both the Guest Contact and Evolve

INTERNET INSTRUCTIONS:
- Instructions for connecting to the wireless internet network can be found inside the Welcome Book upon arrival

ADDITIONAL INFORMATION:
- This property requires stairs for access
- This property does not offer air conditioning
- Please note that, as this is a log home, the upper level has some low-clearance areas

PAGOSA LAKES AMENITIES:
- Only approved and confirmed guests staying at the cabin are eligible to use the amenities. A form must be completed. You will find an authorization form signed by the owner in the guest welcome book. Please fill in the name of each registered guest that will use the amenities. Take the form to the Pagosa Lakes Rec Center to register for fishing permit or rec center use. Proof of ID may be required.
- Only small, manually powered boats (kayaks, rafts, etc.) or small boats with electric motors are allowed on Lake Pagosa

SNOW RESPONSIBILITIES:
- Please be aware that mountain weather is unpredictable and can result in dangerous conditions. Guests are responsible to take all reasonable and responsible precautions as listed below.
Guest's responsibility:
- Snow removal of 6 inches or less on the driveway
- May use the provided snow shovel and ice melt
- Removal of snow on concrete landing and stairs and application of ice melt to ensure walkways are clear of ice and snow
- Inform the Guest Contact if assistance is needed
- In certain situations, snow plowing may be delayed due to weather. The plowing of county roads is not under the control of the Homeowner

PARKING:
- 4 vehicles can be parked in the driveway
- Garage parking is not available for guests at this time; it is used for homeowner storage and trash bin storage
- No parking on county roads

This Rental Agreement Addendum includes additional rental terms in addition to the primary Rental Contract that is required at time of booking. At the sole discretion of Guest Contact or Property Owner/Manager, any violation of these terms may result in immediate termination of rental agreement and forfeiture of rent, deposits. Additional fees will be imposed on Tenants if violations are incurred and damages ensue.
Tenants agree to the following Terms and Conditions

Guest Manual

Agree to read the on-premise Guest Manual in its&rsquo; entirety and notify Guest Contact or Property Owner/Manager of any&nbsp; questions


General

Agree to immediately contact Guest Contact first, as listed in the pre-stay email, or Property Owner/Manager second, in the event of a property issue
Agree to contact 911 in the event of a property emergency.
Agree to leave garage key on kitchen countertop
Agree that upon leaving property to set Fireplace and Baseboard thermostats to 55 (see Guest Manual for operation).
Agree to turn propane gas grill container to the off position (counterclockwise).
Agree to unplug small appliances (coffee maker, toaster) and leave on kitchen counter.
Agree to turn off all lights.
Agree to strip dirty linens.
Agree to leave dirty linens and towels on floor.
Agree not to use garage for parking cars. Cars must be parked in driveway.
Agree to maintain the premises in good, clean and habitual condition and use the premises in a careful and lawful manner.
Agree to lock all doors when leaving property (including garage)
Agree to leave the property in ready-to-rent condition at the end of the agreement, minus housekeeping and cleaning services.
Agree that no Claim will be pursued or Refund requested in the event of an Internet, Electric, Dish TV outage. Mountainous areas are subject to intermittent outages.
Agree to familiarize guests with location and use of Smoke Detectors, Fire Extinguishers, Carbon Monoxide detectors
Agree to notify Guest Contact immediately with any questions or concerns regarding use of Smoke Detectors, Fire Extinguishers, Carbon Monoxide detectors
Agree to familiarize Tenants with wildlife safety and act in a manner consistent regard for wild animals.
Agree to familiarize Tenants with water safety precautions, warning signs of water conditions on lakes, and safety procedures for use of pool or lakes, as necessary
Agree to confirm lakes where swimming is allowed and prohibited
Agree that Landlord has the right to inspect the premises without prior notice at any time to enforce the terms of this agreement.
Agree not to sublet the property or allow more than the total number of occupants to sleep or reside on property during rental
Agree and expressly acknowledge and agree that this agreement is for transient occupancy of the property, and that Tenants do not intend to make the property a residence or household.
Agree that there shall be no refunds of rents due to shortened stays or ruined expectations because of weather conditions.
Agree to purchase a Fishing Permit to fish in Pagosa Lake community lakes, if applicable
Agree to behave in a respectable manner and not disturb other owners or renters in surrounding properties.
Agree that creating excessive disturbances shall be grounds for immediate termination of this agreement, forfeit all rents paid
Agree to pay for any and all Township fines and legal fees incurred due to disturbance
Agree that outdoor noise should be kept to a minimum.
Agree to use the property for legal purposes.
Agree to indemnify and hold harmless the Property Manager/ Owner against any and all claims of personal injury or property damage or loss arising from use of the premises regardless of the nature of the accident, injury or loss.
Agree that that any insurance for property damage or loss which the Property Owner/Manager may maintain on the property does not cover the personal property of Tenants; Tenants should purchase their own, if such coverage is desired.
Agree to not access any locked owner only areas (closets upstairs, closet under stairs)
Agree to pay all reasonable costs, attorney's fees and expenses that shall be incurred as a result of enforcing tthis agreement.



Fire Hazard Mitigation

Agree to use common sense with respect to Fire Dangers
Agree to contact local township for current Fire Danger Levels and restrictions
Agree to not leave or store any objects within 30 inches above or around baseboard heaters, due to fire hazard.
Agree not to smoke inside premises.
Agree smoking outside is permitted as long as:

Agree to COMPLETELY extinguish all smoking materials in a container suitable for hot ashes
Agree to ensure all smoking materials are cooled before being disposed of
Agree to dispose cooled ashes and smoking materials outside in the garage trash container
Agree that tenants are responsible for paying any damages related to smoking or fire
Agree to act in accordance with Property Owners Association rules and regulations; any act that is in direct conflict with POA is not permitted.





Trash

Agree to not leave trash outside at any time.
Agree to utilize trash bags in pantry to contain trash.
Agree to place trash bags in 30 gal trash container in garage.
Agree to keep garage door securely closed at all times.


Property Access

Agree to return all keys. Any missing keys will be charged a $75 non-refundable deposit.
Agree to lock property at all times when away from the premises.
Agree not to give access code to anyone that is not an approved tenant during the stay.


Pets

Agree to not allow animals or pets of any kind to be brought onto the premise unless advance approval is in place and approval accompanies a signed Pet Addendum and collection of a Pet Fee and Pet Rent.

Snow Responsibilities:- Note: Please be aware that mountain weather is unpredictable and can result in dangerous conditions. Guests are responsible to take all reasonable and responsible precautions as listed below.Guest's responsibility:- Snow removal of 6 inches or less on the driveway- May use the provided snow shovel and ice melt- Removal of snow on concrete landing and stairs and application of ice melt to ensure walkways are clear of ice and snow- Inform the Guest Contact if assistance is needed- Guest is responsible for incurred fees for additional snow removal in excess of what is covered by the Homeowner- In certain situations snow plowing may be delayed due to weather and that plow of county roads are not under control of the HomeownerGuest Contact is responsible for:- Coordinating and paying for snow removal for new accumulation above 6 inches- Providing snow shovel and ice melt- Providing snow plowing service contact number to guests- Coordinating the removal of snow from the roof in the event of heavy accumulation- The Guest Contact reserves right to access the property during guest stay to service property which includes, but is not limited to, removal of snow and ice off of driveway and/or roof, as specified above- Make reasonable attempts to contact guests and advise of the need to access property- Disclaimer: The Homeowner/Guest Contact cannot guarantee how quickly snow removal will take place but will make every reasonable effort to schedule service as soon as snow removal service is available and able to access main county roads. Snow removal may be delayed with large amounts of snowfall
Tenants agree to hold harmless the Property Manager/Owner, Guest Contact for damages that arise from breach of any of the above Terms &amp; Conditions in the Short Term Vacation Rental Agreement Addendum or Terms &amp; Conditions in the Standard Evolve Rental Agreement. &nbsp;Should the Tenants violate any of the terms of this agreement, the rental period shall be terminated immediately at the sole discretion of the Guest Contact or Property Manager/Owner.
